Register doctor access to the laboratory portal.

Phone is the login username
The portal normalizes the phone number and uses it as the doctor username.
Lab doctor link
If the doctor already exists in the lab registry, the account links to the existing Lab Doctor ID.
Doctor role required
The account is added to the Doctor role so it can access patient files, referrals, and results.
